We will ask you a few questions:
- Name
- Date of birth
- Reason for your visit
- Insurance information or self-pay information
- An email address to send intake forms
- Your preferred pharmacy
We'll take it from there! We can schedule your visit at your earliest convenience.
You will receive an invitation to join the patient portal where you'll have access to your visit notes, lab results, etc.
For Directions:
From I-20, take Campus Drive North, turn L into the Resource Connection Campus. Turn R on Circle Drive (blue arrows), and follow the drive all the way around until you see Parking Lot E. Enter the 2300 building from the "East Entrance" and GHT is on the hallway to your L.
